Maybe not quite what you want to hear, but see if it makes any sense from a female point of view (tho I'm not into BDSM that way)...
I had a problem with a slipped disc which gave me a cramp-pain in my shoulder: there is a device called 'tens' which uses electro-shocks to relax muscles. It feels like a needle stabbing at the point where the pad is attached.
